Please complete the form below, which also includes a waiver & media release agreement. You will receive a "What to Know Before You Know" email closer to the date of your shift with all the details from arrival to departure & everything in between!

Waiver

Liability & Release Waiver
As a volunteer for Shoes and Clothes for Kids (SC4K), its programs and affiliates, I hereby release and waive liability against SC4K, a nonprofit corporation, its directors, officers, employees, and agents, its successors and assigns, for any injuries or illness that I myself or my dependent(s) may suffer in connection with any volunteer work for SC4K. Further, I agree that SC4K is not liable for any damage to my property or that of my dependent(s) resulting from volunteer work for SC4K. I agree that this release is as broad and inclusive as permitted by the laws of the State of Ohio.

Photo & Media Release
I grant permission to SC4K and its representatives to capture and use my images and/or likeness and that of my dependents in all audiovisual formats, without further notice or compensation. Audiovisual formats include, but are not limited to, photographs, digital media, drawings, renderings, voices, sounds, recordings, and accompanying descriptions. SC4K will own exclusive rights to this content, including but not limited to organization-sponsored websites, publications, promotions, broadcasts, advertisements, and posters. I waive the right to approve the final usages and content and acknowledge SC4K’s ownership. I release SC4K and its representatives, including any firm authorized to publish, broadcast and/or distribute a finished product containing the images, from any claims, damages, or liabilities associated with the use of this content.
